In Case of an Emergency
If you experience a medical discomfort related to your treatment at Restoration Vein Care,
please call (734) 712-4310. Your call will be answered promptly during business hours. During off hours a physician is on-call to speak with regarding your concerns.
Payment
Payment for treatment is required at the time of service. Cash, Check, Visa, MasterCard, Discover, and American Express are accepted. Upon Payment, you will be given a receipt that includes all of the necessary information for you to submit to your insurance carrier for reimbursement on covered procedures.
Insurance Coverage
Insurance coverage varies according to a patient's carrier and medical diagnosis. Treatment of a cosmetic nature is not covered by insurance. We will bill patients covered by Medicare, Priority Health, Blue Care Network and Blue Cross/Blue Shield plans. All others we will work with you regarding the appropriate information to submit to your insurance company for medical reimbursement on necessary medical procedures.
Cancellation Policy
If you find it necessary to cancel an appointment for sclerotherapy, please contact our office at least 24 hours in advance. This will allow an alternate appointment to be rescheduled at your convenience. Your consideration of your physician and the treatment requirements of other patients regarding cancellations is sincerely appreciated.
